Cook a River Pearl Mussel into Boiled River Pearl Mussel, and, very rarely, you will get a pearl in addition to a boiled mussel.

Some players have suggested the chance of obtaining pearls may be related to their Nature belief, while others have claimed that there is a flat 1% chance to obtain a pearl.
NOTE: Loftar stated on IRC on the 27/8/2010 that "No", obtaining a pearl does not depend on your nature/industy belief.
A pearl's quality is equal to the uncooked mussel's quality, unmodified by cauldron, water, or cooking skill.
